A prior stroke and the Medicare GLP-1 Bridge

Short answer: A previous stroke is a Tier 3 qualifying condition: with a BMI of 27 or higher, it makes you eligible for Medicare's $50/month GLP-1 Bridge program, regardless of how long ago the stroke occurred. As with all Bridge criteria, your provider documents it in the prior authorization.

What counts as a stroke for this program

A documented stroke — ischemic or hemorrhagic — in your medical record meets the criterion. The published criteria refer to stroke; whether a TIA ('mini-stroke') in your history satisfies the requirement is a documentation question for your provider, since charts sometimes record events differently than patients remember them. If you were hospitalized, had imaging, or did rehabilitation, the record almost certainly establishes it.

The bigger picture for stroke survivors

Weight management after a stroke reduces the risk factors — blood pressure, blood sugar, vascular disease — that drive second strokes, and GLP-1s have shown cardiovascular risk reduction in people with established vascular disease. That's the logic behind placing stroke history in the lowest-BMI tier. Talk to whichever provider manages your stroke follow-up; they can judge appropriateness and submit the prior authorization in the same visit.

Common questions

Does a TIA (mini-stroke) qualify?

The published criterion is a prior stroke. Whether your chart documents your event as a stroke or a TIA is the deciding factor — ask your provider what your record shows.

How recent must the stroke be?

There's no time limit in the criteria. A stroke from any point in your history counts if documented.

What BMI do I need with a prior stroke?

27 or higher — the program's lowest threshold, about 167 pounds at 5'6" or 188 pounds at 5'10".
